Request forms

Please note, there is a time delay when mailing or faxing forms. Please allow 5-7 business days for processing. These requests must be received by the 10^th of the month to accommodate the requested start date.

You are strongly encouraged to complete any transactions for services by paying fees through myAU. If you cannot and must submit the PDF by email, call 1-800-788-9041 to be transferred to Enrolment Services to provide your credit card information over the phone.

When submitting a course registration, and to accommodate your start date, please ensure your paperwork and full payment is received no later than the 10th of the month. Be sure to declare any necessary prerequisites or have professor approval in place (if required) by the cut-off or you will be registered for the next available start date.
